The RCMP produces forms in Portable Document Format, which are fillable and savable for your convenience.
Filling and saving PDF forms
To be able to fill in and save a PDF form, download and then open the form using the free Adobe Reader:
- Save the PDF fillable or savable form in a folder that you will easily find on your computer.
- Launch Adobe Reader.
- Browse to the folder where you saved the file, and open it from within Adobe Reader. You can now fill and save your PDF form.

RCMP fillable or savable PDF forms have been tested for compatibility with Adobe Reader 9 and higher and may not be fully supported in other PDF viewers and on some mobile devices.

Use this form if you are an Indigenous person who is reclaiming your Indigenous name, and you are required by your province or territory to get a fingerprint-based criminal record check. This form must be submitted at the time of fingerprinting for a criminal record verification to have the $25 administrative fee waived. |

Use this form if you were given a discharge sentence prior to July 24, 1992. If you received a discharge sentence on or after July 24, 1992, your information is automatically removed from Canadian Police Information Centre. However, per Section 6.2 of the Criminal Records Act your discharge sentence will be held separate and apart in the National Repository of Criminal Records for identification purposes. |
