Forms
The RCMP produces forms in Portable Document Format, which are fillable and savable for your convenience.
Filling and saving PDF forms
To be able to fill in and save a PDF form, download and then open the form using the free Adobe Reader:
- Save the PDF fillable or savable form in a folder that you will easily find on your computer.
- Launch Adobe Reader.
- Browse to the folder where you saved the file, and open it from within Adobe Reader. You can now fill and save your PDF form.
Note
RCMP fillable or savable PDF forms have been tested for compatibility with Adobe Reader 9 and higher and may not be fully supported in other PDF viewers and on some mobile devices.
Payments must be made by certified cheque or money order. We are unable to accept cash payments.

For use by members of an Indigenous community (First Nations, Inuit or Métis) who engage in traditional hunting practices. Apply online if a first-time applicant and seeking a firearms licence under the Aboriginal Peoples of Canada Adaptations Regulations (Firearms). You must also submit form RCMP 5592 (Possession and Acquisition Licence) or form RCMP 5485 (Minor’s Licence). |

Print this form on 8.5" x 14" paper (legal size) and complete it before arriving at the border. Do not sign the form until you are in the presence of a Canada Border Services Agency officer. To transport any restricted firearms, you must have a valid Authorization to Transport. You can apply online, contact the Chief Firearms Officer of jurisdiction in the province or territory you are visiting, or submit an application by mail using form RCMP 5490. |
