Submit the forms

After you've initiated your application, you will need to submit the required forms and documents.

The Regular Member Application Questionnaire (RMAQ) is a detailed questionnaire that provides an in-depth look at your past actions and behaviours. Complete the RMAQ honestly and with as much detail as possible. The Suitability Screening form captures personal information and some high-level background information.

Submit a resume and cover letter

Email your cover letter and structured resume that outline your policing experience, competencies and training to the EPO Coordinator.

Complete the vision and hearing exam

Complete the following hearing and vision exams. You can submit your reports when they are requested. The results of these examinations will be valid for 2 years. The costs for the vision and hearing exams are the responsibility of the applicant and will not be reimbursed.

Scan and submit identification and vital records

Scan and email the originals of the following documents to the EPO Coordinator:

  • Canadian birth certificate (front and back), Canadian Citizenship card or permanent resident card and Confirmation of Permanent Residence (IMM 5292)
  • social insurance number
  • marriage, divorce, separation certificate
  • name change certificate (if applicable)
  • 2 passport photos
  • proof of education: Canadian high school transcript showing graduation, or equivalency assessment valid Canadian driver's license (front and back)
  • driving record from every province or territory that you have lived in for the last 3 years
  • provincial health card

Submit your policing records and certificates

Email a copy of the following records to the EPO Coordinator:

  • disciplinary record
  • basic police training program certificate
  • other police training certificates
  • standard first aid certificate with C (CPR) for infants, children, and adults that is valid at the time of training
